duplicate <code> tags

I'm getting duplicate <code> tags with the ones that have CodeBefore and CodeAfter, e.g.

<strong><code><code>UserType</code></strong>

This is problematic since the second <code> tags are not being closed.

Test case, please.

It's not obvious from the .ini where the second tag is coming from. So we'll need the usual test case, a one page doc showing the problem with the .fm, both .inis, .prj, .mif (very important!), and all outputs including the log file, all in one .zip. Be sure to set DeleteMIF=No before you run it. Thanks!
